The Viterion 200 telehealth monitor is a user-friendly device that electronically transmits patients’ vital signs taken at home to their HCP in another location.. Providers can access the system anywhere by using a web browser to monitor their patients and then send messages and advice before patients require more expensive hospitalization or emergency room (ER) visits. of Veteran Patients with Chronic Conditions
Between July 2003 and December 2007, the Veterans Health Administration (VHA) introduced a national home telehealth program, Care Coordination/Home Telehealth (CCHT). Its purpose was to coordinate the care of veteran patients with chronic conditions and avoid their unnecessary admission to long-term institutional care.
VHA’s experience is that an enterprise-wide home telehealth implementation is an appropriate and cost-effective way of managing chronic care patients in both urban and rural settings.

Viterion TeleHealthcare Announces Agreement with the Visting Nurse Service of New York to Provide Telehealth to Home Patients at Risk for Rehospitalization
Viterion TeleHealthcare announced that it has been selected by the Visting Nurse Service of New York (VNSNY), the largest not-for-profit home health agency in the U.S., as a major supplier for state-of-the-art telemedicine devices for telehealth services to help prevent the rehospitalization of as many as 2,700 home health patients in the New York metropolitan area. VNSNY began to roll out its telehealth program in Bronx and Westchester on June 21, with Nassau county and the remaining NYC boroughs to follow over the next year.
